I want you to get the most out of your appointment with me. In order to accomplish this, it may be helpful to review your condition or injury on my website and write down any questions you may have.

I will perform a thorough evaluation at our visit together and to assist me in this, I ask that you provide information online ahead of your visit and also review the information below on what to bring to your appointment.

For your convenience, we have partnered with an online platform called Phreesia to speed up the check-in process.

Here’s how Phreesia works:

  • A few days before your appointment you’ll receive a text message or email from Phreesia (our new check-in platform).
  • Click the link provided and follow the simple on-screen prompts.
  • From here you can give us important information prior to your appointment, update your medical information, or pay your copay.
  • When you arrive, simply let our front desk team know and we’ll be out to call you back for your appointment shortly.

If you have completed Phreesia ahead of time, please arrive 10 minutes ahead of your scheduled appointment and simply let the front desk know that you are here. If you choose to complete your information in our lobby on the day of your appointment, please allow 30 minutes before your appointment to do so.

What to bring to your appointment:

  • Any X-rays, MRI scans, or other diagnostic tests related to your condition or injury. Please bring the actual X-rays/scans in addition to the reports. CD format is preferred.
  • Operative reports of your previous surgeries for the condition or injury for which you are being seen (this is commonly called an "op note." It is a part of your medical record and can be requested from your previous surgeon's office).
  • Any notes of previous evaluations for the condition or injury for which you are being seen (Of note: we are on EPIC and have access to all notes in the Swedish system so you do not need to bring these with you).
  • For patients with a shoulder injury/condition, please bring or wear a tank top or sleeveless shirt to allow for a complete shoulder evaluation.
  • Your photo ID, your current insurance card, and a credit card for copay payment if necessary.

 Phreesia FAQs


What is Phreesia?

Phreesia is a mobile check-in service, allowing you to complete pre-registration forms from your own device (any smartphone, computer, or tablet) in the privacy of your home instead of when you arrive at your appointment. You may also register when you arrive at the office on your own device or on a provided PhreesiaPad (please arrive at least 30 minutes before your appointment if you choose this option).

Why is OPA partnering with Phreesia?

We are always looking to improve the total patient experience. Phreesia will help save you time and ensure that your health records are always up-to-date. The information you enter is private and secure and will allow our team to focus on providing optimal patient care.

Is the Phreesia system secure?

Yes. Phreesia provides industry-leading privacy and security for our patients’ data. They are held to the same standards as OPA related to protecting your information. For additional information about Phreesia’s security click here.

Do we need to use the new system for every appointment?

Yes. But once you have completed the initial registration, the next time will be much quicker because Phreesia saves your answers.

Do I need to download an app to do mobile check-in?

No. There is no app – you simply click the link provided in the text message or email. This allows you to securely answer any registration and medical questions necessary for your appointment.

Can I complete the forms in a language other than English?

Yes! Mobile registration is available to be completed in Spanish or English. (Additional languages to be determined).

What happens if I can’t finish my registration before the appointment?

That’s okay! If you have not completed your registration before the visit, see one of our receptionists, and they will be happy to send you a new link. Don’t worry, Phreesia saves the information you entered previously. Please arrive 30 minutes prior to your appointment time.
